A complaint must include:
Your name, address, telephone number, and email address.
If you are reporting on behalf of an individual, it’s important to provide:
o Individual’s full name
o Date of birth
o The name of their County
A detailed description about your complaint along with any other pertinent information to support your complaint. That includes dates, the provider’s name, if applicable, name of persons involved, etc. If you submit your complaint on-line, you will be able to attach documents to support your complaint.
